  • You are responsible for charges not covered by insurance, including co-pays and out-of-network services.
  • Unpaid balances may result in suspension of services until payment is received.

7. Insurance Coverage

  • We may assist in submitting insurance claims, but coverage varies by provider.
  • Patients must confirm their insurance eligibility before treatment.
  • Pro Balance Acupuncture, P.C. is not responsible for denied claims or insurance disputes.

8. Cancellation and Refund Policy

  • A minimum of 24 hours' notice is required for cancellations or rescheduling.
  • Missed appointments without notice are non-refundable.
  • Refunds for prepaid sessions, packages, or herbal products are at our discretion and may only be granted if unused and in original condition.
